Alternative formats of all items (e.g., audio, large print) and assistance will be made available upon request. If you believe you have been discriminated against based on a disability, you may file a complaint. Complaints should be submitted in writing and include::

  • A description of the alleged discrimination
  • The date and location of the incident

Please submit your written complaint (via email or USPS), within 60 days of the incident, to:

MMDC’s executive director, or an appointee of the executive director, will respond within 15 business days to discuss your complaint as well as any potential resolutions. If, after MMDC’s response, you are unsatisfied with our proposed resolution, you may contact the Great Lakes ADA Center at adagreatlakes@uic.edu or (312)413-1407 (V/TTY) to discuss any additional steps to the resolution of your complaint.
